We are SO excited to have you be a part of our Kids & Youth Ministry this school year!

Information received is confidential and is being gathered for the purposes of serving your child(ren) while in the care of Lakeside Church. Any medical information collected here serves to authorize Lakeside Church, and its staff and volunteers, to obtain medical assistance in case of an emergency. In the case of custody agreements, please include the proper form authorizing parental contacts in an email or in person to Rose Rabidoux (rrabidoux@lakesidechurch.ca).

Partnering with Parents

One of our goals is to partner with parents in supporting your child's spiritual and personal growth not only on youth nights, but also in their day-to-day lives. By sharing their cell number, you give us permission to reach out occasionally with encouragement, reminders, or follow-up from youth nights. We use this as an opportunity to support, pray for, and build healthy, trusted relationships with your youth throughout Grades 6–12.

We like to capture special moments at youth to share on our website, newsletter, and/or social media from time to time. We will always do our best to make sure the photos are positive and encouraging. We will never post any photos of your student that are embarrassing or disparaging.

Student Ministry Transportation (YOUTH ONLY)

We ask for permission to transport your child to and from Lakeside for community events. If our ministries are going outside of the Guelph community, further forms will be sent out for transportation permission. Our drivers will have their G licenses and be over the age of 20 years old, to ensure maturity and safety to and from events. In accordance with our best practices, a student will never be alone with an adult leader in a vehicle.
